Injuries
- Player insurance is compulsory from ESFA.
- This policy is chosen and taken out by FNSW (Football NSW).
- This policy will cover some things but will not be as comprehensive as private cover.
- You can download the accident information and claim form below.
- When you have completed the player section, scan in this copy and email to info@qpfc.org.
- If you do not have access to a scanner, contact info@qpfc.org and arrange for a club official to sign the form in person.
